Network Case Management Services

Network Case Management Services assist clients in accessing the services, treatment and support that they may require, to help facilitate transition from hospital to home, both during and after the rehabilitation phase. With Network Case Management, you have a health professional to guide you every step of the way through your rehabilitation, and to work hand in hand with you and your family to build your capacity to manage your personal circumstances.

We work in partnership with, and empower the individual, their family and all relevant parties to develop and implement a rehabilitation programme and care plan which meets ongoing treatment needs and provides for regular outcome based reviews.

Our organisation is an established NDIS provider. Our ethos is to bring a rehabilitative model to the NDIS sector. Rehabilitation is another name for capacity building or restoring an individual’s health or abilities – resulting in you meeting the goals you have set to achieve. Our services are coordinated by health professionals, and we may assist in areas such as:
